Service Coordination
For the NHTD and TBI Waiver Programs
The Service Coordinator assists the prospective participant in applying for the Waiver Program. They help coordinate and monitor the provision of all services in the Service Plan. Services may include Waiver Services, Medicaid State Plan services, non-Medicaid federal, state and locally funded services, as well as educational, vocational, social, and medical services. The goal is to increase the participant’s independence, productivity and integration into the community while maintaining the health and welfare of the individual.

Community Integration Counseling (C.I.C.)
Our therapists provide in home counseling for participants, and with the recent COVID 19 Pandemic we are able to work using Telehealth, Zoom, facetime, or with phone call sessions. Our Therapists are LMSW level and have years of experience working with various clients on the difficulties that come with new limitations. We provide both one on one and family therapy through both the TBI and NHTD Waiver Programs.

Our E-MOD department can work with you and your Service Coordinator to get the modifications your home needs for you to live independently and comfortably. From appointments for the SCOPE of work to the finished project, our EMOD professionals and contractors will provide licensed, professional work to ensure that you or your loved one is able to continue to live at home..

CTS-Community Transitional Support
Available to those on the TBI/NHTD Waivers who are moving home from a nursing home or rehab.
We can provide the funds to get clients into their own home. If you or your loved one needs financial assistance to enable moving companies to get your things home, we are here to help! Contact us today to get moving!

Consulting Services
We offer a range of consulting services for seniors. From assistance with special needs trusts, to applying for Nursing Homes or touring Assisted Living Communities, we can help you make the important choices in front of you. We can provide answers to questions regarding Medicare, Medicaid, etc.
